Key rules of participation and eligibility:

• There will be age brackets for participation for ages 18-29, 30-39, 40-49 and the last group will be for ages 50 and older. The age will be based on day of event which is October 2, 2010. Winners may be asked to show proof of age prior to being crowned champ for their division or overall champ.

• We will recognize both age division and an overall champion for both men and women in the age groups mentioned above. There will be one winner separately for men and women per age group and one overall male winner and one overall female winner.

• There will be some light variations on some of the events between the men and women's events depending on the event, i.e., field goal for the women will be a shorter distance (optional).

• The event must be performed by the same individual for the entire 15 events in order to be eligible for the division or overall awards, no substitutions permitted during the official start of one participant.

• The cost of participating in the event will be $25 per person or $20 per person on a team consisting of a minimum of 10 participants which will benefit United Way of Southern Kentucky. All individuals who pre-register with forms and payments received prior to October 2, 2010 will also receive a free event tee-shirt on the day of the event while supplies last – sizes not guaranteed. NO REFUNDS.

• Participants will be eligible to complete the event for a second time (with a maximum of 3 times), if they feel strong enough to do so, but can't combine certain events to improve overall score between the multiple attempts. An additional $20 fee can be paid for two additional attempts through the course.

• The winner for each age division will be based on the overall completion time of the entire 15 events of the course. The overall winner will be based on the fastest completion time between all four age divisions for both the men and women.

• This event will occur RAIN OR SHINE with no refunds.

RELEASE AND WAIVER OF LIABILITY

United Way of Southern Kentucky is requiring that the following responsibility clause be read and signed by the undersigned participant. The completion and return of this form is a requirement for participation in United Way’s “Wipeout” competition.

The undersigned acknowledges that he/she has read and understands the “Key rules of participation and eligibility” which are printed on the reverse of this form this Release and Waiver.

The undersigned acknowledges that he/she has taken all appropriate steps and actions to review and assess his/her physical condition and qualification to engage as a participant in United Way’s “Wipeout” competition and that having done so, he/she desires to participate in the United Way’s “Wipeout” competition.

RELEASE

The undersigned, in consideration of, and as a condition to, his/her acceptance as a participant in United Way’s “Wipeout” competition, does hereby release and discharge United Way, United Way of Southern Kentucky, its Board of Directors, its agents, affiliates, officers, and employees from all claim, demands or damages which may arise from loss or injury of any nature to the person or property of the undersigned as a result of participation in said program and agrees to indemnify and hold harmless said parties from any and all loss, damage, or expense incurred as a result of said participation.

I acknowledge that I have carefully read and understand the above statements and agree to the terms and conditions of this release and waiver.
